Photo ID (Driver's License, Passport, Student ID).

Copies of W-2s, 1098, and 1099s forms.

Other records of income.

Bank account and routing numbers for direct deposit.

 

For Foreign Students:

Social Security Card or ITIN - No assistance will be given without it.

Passport AND Visa papers.

Copies of W-2, 1098, 1099, and 1042-S forms.

Other records of income.

Bank account and routing numbers for direct deposit.

Last available paycheck - If you access your paycheck online, you don't need to bring the print out.
